Because It is important we have a clear understanding about our work together, please read the agreement carefully before signing. Please also complete the Intake in this form as well as the Client History. 

Nature and Length of Sessions:  I am committed to results and having each session be meaningful and productive. I ask you to take responsibility for creating value and results for yourself. I see myself as a coach and it is primarily up to you to do the work (and apply what you have learned in your life), to ask for what you want each session, and give me feedback about what works or doesn’t work for you and if there is any more effective way I can assist you.

I will normally request an initial verbal contract about the nature and length of our work together.
